The Quality Record Review (QRR) process is a structured, proactive method for evaluating participant records to ensure that services are well-documented, person-centered, and aligned with regulatory and accreditation standards. At its core, QRR is about supporting service excellence by ensuring that records accurately reflect the care being provided and support participant outcomes.
A well-maintained participant record is more than a compliance artifact—it’s a roadmap of support and a record of transformation. Quality documentation:
Tells the story of each participant’s journey.
Ensures services are responsive to the individual’s needs and preferences.
Protects the agency and participants legally and ethically.
Provides a basis for outcome measurement and program improvement.
Demonstrates conformance to CARF accreditation standards and funding requirements.
The findings from Quality Record Reviews:
Are shared confidentially with supervisors and program leadership.
Inform coaching, training, and quality improvement plans.
Help identify systemic barriers, documentation trends, or gaps in service alignment.
Contribute to readiness for CARF surveys, HUD audits, and other external reviews.

The CARF 2024 Standards Manual emphasizes quality assurance, documentation practices, and person-centered care across various sections. Quality Record Review supports conformance with CARF standards in areas including, but not limited to:
1. Person-Centered Planning
CARF Standard: “The organization ensures that each person served is actively involved in planning and decision-making regarding the services they receive.”
QRR Focus: Reviews assess the presence of clearly documented participant goals, involvement in planning, and reflection of preferences in the service plan.
2. Service Coordination and Delivery
CARF Standard: “The organization coordinates and integrates services to meet individual needs in a timely and consistent manner.”
QRR Focus: Reviews check that documentation supports continuity of care and that case notes correspond to the services outlined in the plan.
3. Assessment and Planning
CARF Standard: “Each person served receives a comprehensive assessment to inform the development of an individualized service plan.”
QRR Focus: Ensures that Housing Stabilization Plans (or equivalent) are individualized, up to date, and revisited regularly based on assessment findings.
4. Rights and Confidentiality
CARF Standard: “The organization protects and promotes the rights of persons served and maintains confidentiality of personal health information.”
QRR Focus: Reviews include verification of HIPAA logs, consent forms, and legal documents required for compliant service delivery.
5. Performance Measurement and Quality Improvement
CARF Standard: “The organization uses a systematic approach to measure outcomes and improve services.”
QRR Focus: Record reviews are part of our larger Performance Management and Measurement Improvement Plan (PMMIP) to support data-informed decision-making.
